By starting your ClearSkinRX consultation you understand and agree:

Nature of telehealth
Telehealth uses electronic communications to share medical information for clinical care. Your clinician may review your history, photos, and other data and may recommend a treatment plan.

Potential benefits
Convenience, timely access to licensed clinicians, and continuity of care.

Potential risks
Technical failures, incomplete data from images or connection issues, and in some cases a need for in-person evaluation.

Alternatives
You may seek in-person care instead of telehealth.

Privacy
Your clinical visit occurs in our Secure Health Portal operated by M & D Integrations, which maintains administrative, physical, and technical safeguards. Do not send medical photos or PHI through this website or standard email.

Prescriptions
A prescription is not guaranteed. If medically appropriate, your clinician may prescribe treatment. Medication is fulfilled and billed by an independent pharmacy. Prescription medication is not sold on this website.

Costs
For the ClearSkin Care Subscription you pay a recurring fee every 90 days on this website. Any prescribed medication is priced and billed by the dispensing pharmacy.

State requirements
You confirm you are physically located in a state where your clinician is licensed at the time of your visit.

Consent
You consent to receive telehealth services and to the sharing of information between ClearSkinRX, your clinician, and the dispensing pharmacy as needed for care and fulfillment.
